Donation Form for Memorial Money, etc. - KPL Foundation (on-line)

Fill out this form to make a monetary donation to the Keokuk Public Library Foundation for current projects and support, or to the Endowment Fund so we can receive your support in perpetuity. Money can also be given in honor or in memory of a loved one. Use this form on-line and then mail your check, or use the PDF, print and fill out, and mail with a check.
